SCORE2 calculator - a modern tool for assessing cardiovascular risk

Cardiovascular diseases (such as myocardial infarction, stroke or sudden cardiac death) are the main cause of mortality in Poland and most European countries. To effectively prevent these events, the European Society of Cardiology (ESC) has developed and implemented an updated risk assessment algorithm calledSCORE2(Systematic Coronary Risk Evaluation 2). It is an advanced mathematical model that allows doctors and patients to estimate the individual 10-year risk of the first serious cardiovascular event resulting in death or requiring hospitalization.

Risk factors considered in the SCORE2 algorithm

The SCORE2 algorithm is based on precise patient data that together influence the health of blood vessels and the heart:

  1. Age and gender:The risk increases with age, and men are statistically more likely to experience cardiac events at a younger age than women.
  2. Tobacco smoking:Tobacco smoking is one of the strongest modifiable factors destroying vascular endothelium.
  3. Systolic blood pressure (SBP):High blood pressure puts a direct strain on the heart and arteries.
  4. Non-HDL cholesterol:Calculated as total cholesterol minus HDL cholesterol. It is a more accurate indicator of atherosclerotic risk than LDL cholesterol alone.
  5. Geographic region:The algorithm classifies countries according to population risk levels. Poland is one of the countries withvery high cardiovascular risk.

SCORE2 risk categories depending on age (for Poland)

According to the guidelines, the obtained percentage result assigns the patient to the appropriate risk category depending on his age group:

How to interpret the result and how to reduce cardiac risk?

The result is given as a percentage and indicates the probability of having a heart attack or stroke in the next 10 years. For example, a result of 12% means that statistically 12 out of 100 people with the same parameters will have a heart attack or stroke within 10 years. If you are classified as a high or very high risk group, a medical consultation is necessary to consider the implementation of pharmacotherapy (e.g. blood pressure lowering drugs or cholesterol-lowering statins) and radical lifestyle modification (Mediterranean diet, regular physical activity, quitting smoking).

What is SCORE2 Calculator?

This is an algorithm recommended by the Polish and European Society of Cardiology for assessing the 10-year risk of a first heart attack or stroke in patients without previous cardiovascular diseases.

How is SCORE2 different from the older SCORE system?

The older SCORE system only assessed the risk of death from cardiac causes. The new SCORE2 system estimates the risk of both death and non-fatal events (heart attacks and strokes experienced by the patient), which provides a more complete clinical picture.

How is non-HDL cholesterol calculated?

It is calculated in a very simple way: the concentration of "good" HDL cholesterol (non-HDL = TC - HDL) is subtracted from the total cholesterol concentration.

Why does age have such a large impact on SCORE2?

With age, the aging process of blood vessels naturally progresses and the influence of atherosclerotic factors accumulates, which makes age the strongest, although unmodifiable, risk factor.

Is the SCORE2 calculator applicable to patients with diabetes?

No. Patients with type 1 or type 2 diabetes, chronic kidney disease and very high individual risk factors (e.g. extreme hypertension) are automatically classified as high or very high risk and do not require SCORE2 calculation.
