Scabies - characteristics of a parasitic skin disease

Scabies is an infectious dermatological disease caused by microscopic mites - the human scabies mite (Sarcoptes scabiei). Female parasites drill small corridors in the stratum corneum (so-called scabies burrows) in which they lay eggs. The main and most troublesome symptom of scabies is persistent itching of the skin, which intensifies at night when the body heats up under the bedding. Infection occurs as a result of direct physical contact with the skin of a sick person (e.g. sleeping in the same bed, holding hands), and much less often through sharing towels, bedding or clothes. This disease can affect anyone, regardless of the level of personal hygiene.

Characteristic symptoms of scabies and location of lesions

Diagnosis of scabies at home is based on the analysis of the appearance of the skin and the location of rashes:

  1. Location of skin lesions:Scabies mites prefer places on the body with thinner skin and higher temperature: spaces between the fingers of the hands, bends of the wrists and elbows, skin folds under breasts in women, around the navel, buttocks and genitals in men.
  2. Scabies burrows:Fine, gray or pink lines, from a few millimeters to a centimeter in length, visible on the epidermis.
  3. Rash:Small lumps, vesicles and crusts resulting from scratching (so-called scabs), which may become infected with bacteria.

Scabies is often confused with allergies or atopic dermatitis. The table below makes it easier to distinguish between these conditions:

What does home diagnostics look like (Ink Test)?

One of the traditional home methods of making scabies burrows visible is the so-called **ink (or iodine) test**:

  1. A drop of dark ink, mascara or iodine is applied to a suspected, itchy skin lesion (e.g. between the fingers of the hand).
  2. After a minute, the skin is gently wiped with a cotton pad soaked in salicylic alcohol or water.
  3. The ink seeps into the cracks carved by the scabies mite. If a distinct, darker line (corridor) remains on the skin, the test indicates the presence of scabies mites.

What are the first symptoms of scabies?

The first symptom is intense itching of the skin, which intensifies after the body warms up (e.g. in the evening, after a bath). It usually appears 2-6 weeks after infection.

What medications are used for scabies?

The basic first-line treatment is a cream containing **permethrin** (5%), which is applied to the entire body for 8-12 hours. Sulfur ointments or oral ivermectin are also used.

Do all household members have to undergo scabies treatment?

Yes. This is an absolute condition for recovery. All family members and sexual partners of the infected person must undergo treatment with an antiparasitic preparation at the same time, even if they do not yet have symptoms of itching.

How to disinfect a house after scabies?

Bedding, towels and clothes should be washed at a minimum temperature of 60°C and ironed with a hot iron. Seal items that cannot be washed tightly in plastic bags for at least 7 days - scabies mites die outside the human skin within 3-4 days.

Can a dog or cat infect humans with scabies?

Animals suffer from scabies caused by other varieties of the parasite (e.g. canine sarcoptic mite). They can spread to humans and cause short-term itching, but they cannot reproduce on human skin and die quickly without treatment.
