Parasitic diseases of the digestive system are a common health problem in children, especially those attending nurseries, kindergartens and schools. The most common parasites in temperate climates are pinworms (causing pinworm), human roundworm (ascariasis) and the protozoan Giardia lamblia (giardiasis). Children are most often infected by having dirty hands (so-called dirty hands disease), putting toys in their mouths at kindergarten, playing in the sandbox without supervision or eating unwashed fruit. Diagnosing parasites can be difficult because their eggs or cysts are excreted irregularly, requiring repeated stool tests.
Our guide describes methods for diagnosing worms in children at home, explains how to properly collect stool samples, what the cellophane tape test for pinworms is and how to prevent reinfections in household members.
Symptoms that may indicate the presence of parasites in a child
Parasites cause a number of symptoms, some of which concern the nervous system due to the toxins secreted by the worms:
- Itching around the anus:The most characteristic symptom of pinworms, which intensifies in the evening and at night, when the female pinworm wanders into these areas. area to lay eggs. This causes the child to fidget in bed and insomnia.
- Behavioral disorders:Hyperactivity, tearfulness, difficulty concentrating, grinding teeth during sleep.
- Gastrointestinal symptoms:Abdominal pain (often around the navel), loss of appetite, nausea, weight loss or failure to gain weight.
- Appearance:Pale skin, dark circles under the eyes, allergic rashes on the skin.
Table: Comparison of the most common parasites in children in Poland
The table below shows the differences between the three leading gastrointestinal parasites in children:
| Name of the parasite / Disease | Main clinical symptoms | First-line diagnostic method | Prevention and treatment |
|---|---|---|---|
| Pinworm human (Portworm) | Persistent anal itching at night, teeth grinding, restlessness, scratching. | Cellophane tape test (swab from around the anus in the morning before washing). | Antiparasitic drugs (e.g. pyrantelum). Washing bedding at 60°C. Deworming the whole family. |
| Human roundworm (Ascariasis) | Abdominal pain, cough (in the stage of pulmonary migration of larvae), loss of appetite, hives. | Stool test for eggs (min. 3 samples taken every few days). | Prescription medications (e.g. mebendazole, albendazole). Thorough washing of vegetables and fruits. |
| Giardia lamblia (Giardiosis) | Chronic diarrhea (fatty, watery), nausea, flatulence, abdominal pain after eating. | Stool enzyme-linked immunosorbent assay (ELISA) for lamblia antigens or examination of duodenal contents. | Drug treatment ordered by a doctor. Taking care of hand hygiene, drinking boiled water. |
How to properly perform a home pinworm tape test?
Threadworm is difficult to detect in a standard stool test because females do not lay eggs in the intestine but on the skin around the anus. The most effective is the **cellophane tape test (NIH test)**:
- The test is performed in the morning, immediately after the child wakes up, **before going to the toilet and before washing/bathing**.
- A piece of transparent adhesive tape (approx. 2 cm wide) is stuck to the skin around the child's anus and pressed for a few seconds.
- The tape is peeled off and stuck flat on a clean glass slide (received from the laboratory) or on clean foil.
- The sample is sent to the laboratory, where the technician looks for stuck pinworm eggs under a microscope. It is worth repeating the test 3 times at intervals of several days.
Frequently asked questions (FAQ)
Does a child's teeth grinding always mean that he or she has worms?
No. Teeth grinding (bruxism) may be a symptom of parasites, but very often it results from emotional tension, stress in the child or malocclusion. However, it is worth ruling out pinworms first.
How many times should the stool test for parasites be repeated?
One stool test gives a reliability of only 30-40%. To exclude the presence of parasites, the stool test should be repeated at least 3 times, taking samples at intervals of 2-3 days within 10 days.
Does Pyrantelum for pinworms work on parasite eggs?
No. Pyrantelum only affects adult pinworms, paralyzing their muscles, which allows them to be expelled from the body. The drug does not destroy eggs, so the treatment must be repeated after 2-3 weeks** when new parasites hatch from the eggs.
How to protect your child from re-infection with pinworms in kindergarten?
It is crucial to cut your child's nails short (eggs do not accumulate under the nail plate), wash their hands with soap after returning home and before each meal, and wash toys regularly at kindergarten. During treatment, bedding and underwear should be washed at high temperatures and ironed.
Is preventive deworming without tests safe?
Doctors advise against prophylactic administration of strong prescription antiparasitic drugs (e.g. Zentel) without confirmation of infection in laboratory tests or without clear clinical symptoms, because they burden the child's liver.
